The Question Behind The Prize
By our forties and fifties, many of us begin noticing changes that are difficult to measure but easy to feel.
A hard workout takes longer to recover from. Energy disappears earlier in the day. A name takes an extra second to arrive. A minor illness can leave us tired for much longer than it once did.
Then we see more serious versions around us. Someone stops traveling because walking through an airport has become difficult. A capable person begins worrying about falling. An ordinary infection becomes a hospital stay.
We give these problems different names: weakness, fatigue, poor recovery, memory loss, inflammation, or immune decline.
But they may be different signs of the same larger problem. The systems that make energy, repair damage, protect us from infection, and keep our muscles and brain working become less dependable together.
Medicine usually waits until one of those systems fails badly enough to receive a disease name. Then it treats that disease.
XPRIZE is asking a more ambitious question:
Could we restore several aging systems before they fail?
What The Teams Actually Have To Prove
XPRIZE Healthspan is a seven-year, $101 million competition.
The goal sounds almost impossible: restore at least ten years of lost muscle, cognitive, and immune function in older adults within one year. The most ambitious target is twenty years.
In plain English, the judges are not looking for a treatment that merely makes a laboratory number look younger.
They want to know whether an older person can move better, think more clearly, and show stronger immune function.
That distinction matters. Longevity research is filled with exciting animal studies and blood tests that move in the right direction. Much less often do we see an older person regain an ability that affects daily life.
In August 2026, XPRIZE advanced twenty teams into the finalist stage. Ten received $1 million each to accelerate development and human testing. The final trials are expected to run through 2029, and the competition is scheduled to end in 2030.
The prize does not prove that any of these treatments work. It creates a shared test that makes the claims easier to compare.
Five Ways The Teams Are Trying To Change Aging
The awardees are taking five broad routes.
1. Help Old Cells Make Energy Again
One team is working on mitochondria, the tiny power plants inside our cells. When mitochondria struggle, muscles tire more easily and recovery can slow.
The hope is simple: improve the cell's energy system and the person may regain some physical capacity.
2. Turn Down Harmful Signals
Several teams are targeting proteins connected to chronic inflammation, tissue scarring, and poor repair.
This does not mean inflammation is always bad. We need it to fight infection and heal injuries. The problem is the low, persistent inflammation that can remain switched on as we age.
3. Deliver Better Instructions
Some teams use extremely small biological packages called extracellular vesicles. Cells naturally use these packages to send messages to one another.
Researchers hope they can deliver helpful instructions to damaged tissue. The big unanswered question is whether enough of those messages reach the right place and create a benefit a person can actually feel.
4. Replace Or Reprogram Damaged Biology
These approaches use cells or gene therapy. Instead of helping an old system work a little better, they try to replace part of it or give the body new instructions.
The potential is enormous. So are the safety, manufacturing, and regulatory challenges.
5. Combine Several Available Tools
Other teams are combining medicines, supplements, exercise, coaching, and detailed measurement.
This may be the fastest route to something people can use. It is also harder to study. If the combination works, we still need to know which part created the benefit.
Does Better Function Mean A Longer Life?
This is the elephant in the room.
Most people are not following longevity science because they want a younger-looking blood test. They want more capable years and, if possible, more years of life.
XPRIZE cannot wait several decades to see which group lives longest. It is testing the part that can be measured now.
Muscle strength affects falls, mobility, and independence. Cognitive function affects our ability to make decisions and remain ourselves. Immune function affects how well we respond when infection or cancer appears.
If one treatment can improve all three, it would suggest that several parts of aging can be changed together.
That would not prove the treatment extends life. It would give researchers a serious reason to run the longer and larger studies needed to find out.
The path is:
Show that important abilities can return.
Show that the improvement lasts.
Show that people remain healthier and independent for longer.
Then determine whether that healthier period becomes a longer life.
The Most Important Thing I Found
After reviewing the awardees, I expected one team to emerge as the obvious leader.
That did not happen.
One approach has the strongest repeated lifespan evidence in normal mammals. A different company has the strongest controlled human evidence for improving a function tied to independence. Another has the most mature drug platform.
The strongest lifespan idea, the strongest evidence in people, and the most advanced medicine belong to different teams.
That is why a public finalist list is only the beginning. The useful question is not simply, "Who made the list?"
It is, "What has each team actually proved, and what result must come next?"
